- I understand and acknowledge any risks or complications associated with the treatment as they’ve been explained to me. I know there may be temporary side effects like swelling, redness, and bumps.
- I understand I should not undergo waxing if I have tanned through any method in the last 72 hours.
- I understand I should not be waxed if I used Retinoids, scrubs, exfoliants, or peels in the last 72 hours.
- I have followed all pre-procedure care instructions as they have been explained to me.
- I understand all aftercare procedures for waxing as they’ve been explained, and I intend to adhere to the instructions given to me.
- I confirm that I have given an accurate account of my medical history, including any allergies or medications that I am currently taking or intend to take.
- I understand that complications are rare but possible. In the event of any complications, I will contact the provider who performed the treatment.
- I understand that waxing can be uncomfortable, causing stinging, heat, and/or tightness.
- I confirm I have not used Accutane in the last year.
- I confirm I do not currently have any open skin lesions or active herpes outbreak.
- If I am getting a Brazilian or bikini wax, I will tell my provider if I am on my menstrual cycle.
- I understand waxing is an elective treatment and not medically necessary.
